Is Your Hospice Pharmacy PPD Really All In?
A hospice pharmacy per patient per day figure is only useful when leadership can see what the number includes, how it was calculated, and what caused it…

Why hospice is different
Standard pharmacy benefit management was designed for the commercial market: volume-based formularies, employer health plans and retail incentives that have nothing to do with how hospice works. Hospices that use commercial PBM models pay for the mismatch in higher PPD, compliance exposure and a formulary that ignores the clinical reality of end-of-life care.
MaxHaven was built from the ground up for hospice. Our formulary was designed by palliative care pharmacists who know the Medicare Hospice Benefit inside and out. Our pricing removes spread and hidden markups. Our data shows exactly where every pharmacy dollar goes.
Hospice leaders deserve to know exactly where their pharmacy dollars go, and exactly what they save.
What MaxHaven delivers
Every MaxHaven partnership rests on the same three commitments: pricing you can verify, a PPD you can see and care standards built for hospice.
Pricing you can count on
You pay the actual cost of drugs plus a clear pharmacy dispensing fee, and every dollar in savings flows to your organization. MaxHaven benchmarks medication prices to NADAC, the public National Average Drug Acquisition Cost, instead of inflated list prices.

Our story
MaxHaven was founded on a simple premise: hospices deserve a pharmacy benefit partner that understands their world. The clinical complexity of end-of-life care, the financial pressure of Medicare reimbursement and the work of managing pharmacy across a dispersed patient population.
We are not a generalist PBM that added a hospice module. MaxHaven launched in 2023 as part of PPOk, the family of pharmacy brands founded in Oklahoma in 1985 to strengthen independent pharmacy. That heritage shapes everything we do: transparency, respect for the organizations we serve and incentives that stay aligned with yours.

The business side of hospice pharmacy becomes manageable when an independent pharmacy can answer three questions: How will we be paid? What is happening…
MaxHaven Rx is a hospice-specialized pharmacy benefit manager (PBM) launched in 2023 by PPOk. It manages prescription drug costs and formulary compliance for hospice organizations: for-profit, non-profit, hospital-based and home-health-based programs. It works on a transparent, no-spread-pricing model with a formulary designed by palliative care pharmacists.
Three ways. First, it benchmarks drug prices to NADAC, what pharmacies actually pay, instead of inflated AWP list prices. Second, it eliminates spread pricing and hidden markups, so you pay the actual drug cost plus a clear dispensing fee. Third, its hospice-specific formulary is optimized to reduce PPD without compromising care; formulary optimization drives 50–70% of total client savings.
PPD stands for per-patient-per-day drug cost, the main metric used to benchmark hospice pharmacy spend. It is the average daily cost of all medications dispensed to a hospice patient. MaxHaven’s average PPD is less than $6.00, compared with an industry average of $11.00, and MaxHaven provides real-time PPD tracking for every client.
The Medicare Hospice Benefit (MHB) covers medications related to a patient’s terminal diagnosis under the hospice per diem rate, so the hospice is financially responsible for those drug costs. MaxHaven’s formulary and compliance protocols are built around MHB requirements, with built-in guardrails and audit support for CMS regulations.
Hospice administrators and executive teams, hospice consortia and group purchasing organizations, non-profit hospices, for-profit hospices, hospital-based hospice programs and hospice start-ups. The contract structure adapts to size, ownership model and patient population.
MaxHaven will benchmark your current per-patient-per-day drug costs against industry standards, at no cost and with no commitment.
